Following the “Five Rights” of Medication Safety

The “Five Rights” of medication safety are guidelines developed by the National Academy of Medicine to help reduce medication errors. Following these rights will help you and your healthcare provider ensure you’re using your medications correctly. Here’s how service members can follow the Five Rights:

  1. Right Patient
    • Confirm Identity: Always make sure any medication prescribed is for you. This involves checking that the prescription labels and any medication package or documentation are in your name.
    • Personal Records: Keep a personal health record that includes your full name and any other identifying information that matches your medical records and prescriptions.
  2. Right Medication
    • Check Prescriptions: Upon receiving a medication, check that it is the one prescribed by your healthcare provider. This includes verifying the name and what the medication is going to treat.
    • Educate Yourself: Understand what each medication is for, how it should work and its potential side effects. Ask your pharmacist or healthcare provider if you are unsure.
  3. Right Dose
    • Review Dosage: Make sure the dosage on the prescription matches what your doctor told you during your visit. If there is any difference, contact your healthcare provider.
    • Measurement Tools: Use the right tools (such as a dosing spoon, syringe, or cup) to measure liquid medications correctly. For pills, make sure you know whether they can be split or if they must be taken whole.
  4. Right Time
    • Stick to a Schedule: Take your medication at the times prescribed by your healthcare provider. This helps maintain a regular level of medication in your body, which can be important for its effectiveness.
    • Reminders: Set an alarm or use medication reminder apps to help you take your medication at the right time every day.
  5. Right Route
    • How to Use: Be clear on how the medication should be taken. Whether it is oral, injectable, topical, or another form, make sure you understand how you are supposed to take your medicine.
    • Follow Instructions: If there are specific instructions such as taking the medication with food, on an empty stomach, or not lying down after taking it, be sure to follow these guidelines.

In addition to the Five Rights, always communicate with your healthcare providers about all medications you are taking, including over-the-counter drugs, supplements and herbal products.
